Study Title: Impact of AZHEALTHTXT health information dissemination and resilience.

Hello! We invite you to take part in our research study about the AZHEALTHTXT platform. This research aims to learn from your experiences to make the platform better. Please read the information below and feel free to ask any questions before deciding if you want to take part. We anticipate enrolling all the subscribers of AZHEALTHTXT i.e. up to 300 subscribers.
The purpose of this survey is to test how well the AZHEALTHTXT platform works on health literacy, community engagement, and healthcare outcomes in Arizona in building resilience. Building resilience is like learning to bounce back from life's challenges, and becoming stronger with each hurdle you face. We want to know what you think about the health information it provides and how it could be improved. Consenting and responding to the survey will take 10-15 min for each participant.
If you agree to participate, you will be asked to share your thoughts and experiences with AZHEALTHTXT. This involves answering questions in a 10-15 minute survey.
There are no physical risks associated with participation in this research. However, there is the potential risk of loss of confidentiality. Because of the nature of the data, it may be possible to deduce your identity; however, there will be no attempt to do so, and your data will be reported in a way that will not identify you. Your name will not be used in any report. Identifiable research data will be encrypted, and password protected. You will not be compensated for participating, and there is no cost to you, except for your time. The benefits of participating in this survey include the opportunity to influence the improvement of AZHEALTHTXT, making it better for you and others in the community.
We will only share your information in an unidentified aggregated manner with others outside of our research team such as our Community Advisory Committee or in future publications.
Participation in this study is entirely voluntary and you do not have to participate. You can choose not to answer any questions or withdraw from the study at any time without any negative consequences. Data collected up to the point of withdrawal will be used for the study.
The information that you provide in the study will be handled confidentially. However, there may be circumstances where this information must be released or shared as required by law. The University of Arizona Institutional Review Board may review the research records for monitoring purposes.
Your information may be used for future research or shared with another researcher for future research studies without additional consent.
