Welcome to our COVID-19 survey brought to you by the University of Arizona Mel and Enid Zuckerman College of Public Health (MEZCOPH)! You are invited to participate in our research study about your experiences during the COVID-19 pandemic. Multiple states have been experiencing fluctuations in the number of COVID-19 cases, and we would like to hear about your experiences to better understand how we can support you best as you continue to battle COVID-19 on the front lines.
The following questions will ask you about general demographics and history of discriminatory incidents with resources at the end of the survey.
Please note: Your responses are anonymous and confidential. This survey does not ask for your name or any other identifying information. If you would like to be entered to win a $25 Amazon gift card, a separate link will be provided at the end of the survey - for this we will only ask for your email address. Study Title: Surveillance of Xenophobia: Driving Factors and Magnitude of Prejudice, Discrimination, and Racial Tension in the Time of COVID-19 - Informed Consent Form (Healthcare/Medical Community Survey)
Principal Investigator: Kacey Ernst, PhD, MPH Medical Community Survey Research Study on the Experiences of Healthcare Workers During the COVID-19 Pandemic in the United States.
Participation is completely voluntary. Your answers will not be provided to your institution – the responses from healthcare workers across all participating institutions will be aggregated into a single report generated by the University of Arizona’s Mel and Enid Zuckerman College of Public Health (MEZCOPH). Your responses are anonymous and confidential. This survey does not ask for your name or any other identifying information. You may skip any question you prefer not to answer and may end the survey at any time. Thank you for your participation!
This is a consent form for research participation. It contains important information about this study and what to expect if you decide to participate. Please consider the information carefully. Background
Racism, xenophobia, and discrimination are prevalent across the world and are issues that have been increasing in incidence in recent times as a result of COVID-19. Coronavirus fears are fueling a rise in intolerance, discrimination, and health inequity. Now is the time to speak out against prejudice and hateful speech.
UNICEF cites five ways to fight racism and xenophobia: Celebrate other cultures, Call out bigotry and hate speech, Teach children kindness and how to talk about differences, Report attacks — intervene if it's safe to do so, Support human rights organizations like UNICEF.
Procedures & Expected Time to Complete
You will be asked to complete the following survey which should take approximately 10-15 minutes of your time. There is no cost to you other than time. You will not be compensated for participating.
Risks
Although the risks are expected to be minimal, you may experience discomfort in reporting difficult life circumstances.
Benefits
We hope that after participating in this study you will have increased awareness of racialize discrimination due to COVID-19 fears, knowledge of firearm safety practices, and community and workplace resources available to aid you.
The information that you provide in the study will be handled confidentially. However, there may be circumstances where this information must be released or shared as required by law. The University of Arizona Institutional Review Board may review the research records for monitoring purposes.
