Individual Studies forms MUST be signed by both you AND your Faculty Advisor by 5:00 PM (AZ Time) on the first day of whatever 5, 7, 10 or 13-week session begins. Please refer to the Dates and Deadlines Calendar for when each of the different sessions begin

Forms submitted after the first day of when your 5, 7, 10 or 13-week session begins will NOT be approved for enrollment.
